Cashmere Health Extended Prescribing Policy
Eligibility:
- Not everyone will be eligible for 6 or 12 monthly scripts – Extended scripts are at the discretion of your registered GP.
- Approval for a 6 or 12 monthly script must be made by your registered GP
- Extended prescriptions are only for stable conditions, unlikely to change within the next 12 months.
- Controlled drugs and medications requiring monitoring are not eligible
- Prescription length depends on how often your condition requires review
Prescribing Doctor
- Your 12 month prescription and all reviews must be completed by the same GP wherever possible to ensure continuity of care
Pharmacy
- Your prescription will be sent to one nominated pharmacy only
- Changing pharmacies will incur an administrative(script) fee, and the prescription will transfer for the remaining duration only
Repeat prescriptions
- Phone or online repeat requests are not permitted for 12 monthly prescriptions
- Repeat prescriptions for a 12-month prescriptions require a 15minute in-person appointment with your registered GP and are for medication review only.
Any additional concerns you wish to have addressed during the consult will require you to make a 30-minute appointment. - If your script gets changed elsewhere (e.g. following hospital discharge), your 12-monthly script will cease, until review with your prescribing GP.
Patient responsibilities
- You must remain up to date with all required screening and blood tests, and complete any follow up investigations requested by your GP
- Failure to complete monitoring or if your condition becomes unstable, the 12 monthly script maybe cancelled.
- Should your condition change or deteriorate you must make an appointment with your GP to discuss.
