Guide to Patient Enrolment
Thank you for your interest in enrolling with Cashmere Health. Please read through the following information. If you have any questions please call our reception team.
In order to be an enrolled patient with us, you need to live within our boundaries and do the following:
- Check you are eligible for NZ health care
- You need to agree to our terms and conditions, policies and terms of trade.
- Each person enrolling with us must complete a separate enrolment form.
- Provide suitable ID and proof of address.
- Complete our online enrolment form.
