Enrolling With Us

Thank you for choosing to enrol with Cashmere Health Practice. We want to make the enrolment process as simple as possible. Please take a moment to read through the following information. If you have any questions, please call our reception team to discuss.

Enrolment Checklist:

  1. We welcome enrolments from families living within our enrolment zone, which ensures we can best serve our local community. If you are unsure whether your address is within the zone, our reception team will be happy to assist.
  2. Check your eligibility for New Zealand health care. There is a comprehensive guide on the Ministry of Health and for more information, visit Te Whatu Ora.
  3. By enrolling with us you accept our terms and conditions, policies and terms of trade. Please read these now:

  5. Each person enrolling with us must complete a separate enrolment form.
    For children under 16 years of age (including babies), a parent or guardian must complete and sign the form on their behalf.
    Anyone 16 years or older is required to complete and sign their own enrolment form.
  6. Proof of identity (ID) is a government requirement for anyone enrolling with a new medical centre.
    If you are an NZ citizen, one form of photographic ID such as a driver’s license, student ID, Kiwi Access Card, or passport is accepted
    Other nationalities will be required to present their passport and visa information as per the Ministry of Health requirements. More information around visa’s can be found by clicking here.
  7. Proof of address is also required, please supply a document such as a power bill, phone bill, WINZ letter etc, which confirms your name and your address as the one shown on the enrolment form.

How to enrol

  • Enrolments must be made using out online enrolment form.
  • Ensure you have proof or ID, proof of address, passport and visa(where applicable) ready to upload. We will not be able to process incomplete applications.
