Health Coach
The Health Coach is a non-clinical role that empowers patients to take an active role in managing their health and wellbeing. They work alongside patients to support self-management, set and achieve goals related to recovery, lifestyle, wellbeing, and behaviour change, and provide ongoing encouragement and practical guidance. Health Coaches also help patients navigate health and community services, connecting them with the social, cultural, and other supports they need to improve their overall health and wellbeing.
Consultations with the Health coach are funded. The initial consult is usually 60 minutes, with shorter follow-up sessions as required.
