We are currently accepting enrolment applications. Please read the information below before submitting your online enrolment form.

Enrolment Zone

We welcome enrolments from families living within our enrolment zone, which ensures we can best serve our local community. If you are unsure whether your address is within the zone, our reception team will be happy to assist.

Completing Enrolment Forms

Each person enrolling with us must complete a separate enrolment form.

  • Children under 16 years of age must have the form completed and signed by a parent or guardian.
  • Anyone aged 16 years or older must complete and sign their own enrolment form.

Prepare Your Proof of ID

Proof of identity (ID) is a government requirement for anyone enrolling with a new medical centre. If you are an NZ citizen, one form of photographic ID such as:

  • Driver's licence
  • Student ID
  • Kiwi Access Card
  • Passport

Other nationalities will be required to present their passport and visa information as per the Ministry of Health requirements. More information around visa’s can be found by clicking here.

Prepare Your Proof of Address

Proof of address is also required, please supply a document such as:

  • Power bill
  • WINZ letter
  • Phone bill
  • etc

which confirms your name and your address as the one shown on the enrolment form.

Ready to Enrol?

Enrolments must be made using our online enrolment form.

  • ✓ Proof of identity ready
  • ✓ Proof of address ready
  • ✓ Passport and visa documents ready (where applicable)
  • ✓ Upload all required documents at the same time

If any documents are missing, you will need to restart the enrolment process. Proof of address must be included for every application.
